App Review usually takes a few days. The download button appears here the moment it clears — no date is promised, because that is Apple’s call rather than mine.

Fair questions about paying for it
Why is there no free tier?
There is no advertising and no data collection, so the app has exactly one way to pay for itself. A single small price keeps that honest rather than turning families into the product.
What happens if you stop working on it?
The app and your records live on your phone. Nothing switches off remotely, and nothing stops working because a server was turned off.
What about cloud sync later?
Sync across phones is a genuinely different product with a running cost behind it, so if it arrives it will be its own optional tier. It will never be required to keep using what you bought.
